Information We Store & Exceptions For Sharing

Alpha and Omega Massage are required by law to store completed consent & consultation forms, treatment records, and business records. These include both personal and health related data. These are essential to providing Massage/Treatment, to ensure compliance with insurance and legal requirements, professional standards, but also to determine if it is safe to provide Massage/Treatment. We will never sell your data, share it (excluding the exceptions below), or send out unsolicited marketing materials. The information we store will comprise of:

Personal data such as your name, address, date of birth, telephone number, email address, invoice details.
Health related data with details of your medical history, health problems, injuries, allergies, treatment dates, SOAP/treatment records detailing your treatment and evaluation thereafter.

We will retain your records for 7 years pursuant to insurance stipulations. We will only share your personal data when legally required to do so with authorised legal, taxation, and regulatory bodies, the insurance company providing professional indemnity, and with professional membership bodies. All records 7 years or older will be destroyed and erased (excluding those required for any exceptional cases e.g. public interest, litigation or insurance obligations).

This site is for mobile and treatment room appointments, occassionally based in other business locations, in sporting venues, rented treatment premises, gyms, nursing homes etc. Therefore, we will on occasion for clinical management purposes, share via encrypted emails and cloud storage, personal data from bookings, consent & consultation forms, and treament records if working within a clinic or other remote location. This data will only be used by the therapist and clinic management to manage records for taxation and legal purposes, to manage conflict of interests, for diary management purposes between mobile and clinic based appointments, and where a referral is needed between services. No information will be shared by any clinic externally.

How Alpha & Omega Massage Store Personal Data

All personal data submitted via this website is stored on a biometric and password controlled laptop. This is backed-up to password protected and encrypted cloud storage. Only Grant Lindsay has access to these records and will take all appropriate steps to ensure confidentiality and safeguarding your data. Data submitted on forms on this website automatically generate PDF files, these are emailed and stored on the cloud in a password protected & encrypted business account. Consultation forms and client notes completed in writing during a treatment will be kept in secure storage at the business address and retained for 7 years in line with legal, regulatory, and taxation purposes.

We reserve the right to modify this privacy policy at any time, so please review it frequently. Changes and clarifications will take effect immediately upon their posting on the website. If we make material changes to this policy, we will notify you here that it has been updated, so that you are aware of what information we collect, how we use it, and under what circumstances, if any, we use and/or disclose it.
